TRIUMPHANT TAIL AWARD

BASE CRITERIA for Triumphant Tail award:

  1. Must be a current member of FAPAA, either a Regular Member or Associate Member.
  2. Must demonstrate the success of an individual or a team effort to go above and beyond for a hard-to-place animal that had been at the shelter for an extended period of time. The team can be internal (an individual or staff team effort), or external (working with other agencies or organizations).

TRIUMPHANT TAIL AWARD NOMINATION FORM:  This form must be included with nomination 2024 FAPAA Triumphant Tail Award

NARRATIVE OF ACCOMPLISHMENTS/PERFORMANCE:  Please include separately, along with any supporting documents, letters of recommendation, etc., and submit with this form.

PHOTOS: Mandatory requirement for all awards. Please include at least two (2) quality JPEG images for use during the awards presentation.

    Narrative of Accomplishments/Performance

    Please include supporting documentation that will help demonstrate the worthiness of your nomination, however please submit no more than 10 pages total per nomination. This may include letters of recommendation, details of their outstanding achievement, details of their exceptional performance, etc.
