LIFETIME ACHIEVEMENT AWARD

BASE CRITERIA for Lifetime Achievement award:

  1. This individual must be currently employed with a FAPAA Regular Member Agency in good standing during the year 2023 or retired from a FAPAA Regular Member Agency during 2023.
  2. Individual must be employed or recently retired from a Regular Agency and spent a minimum of 20 years making consistent and regular contributions to improving animal welfare, animal protection, and/or advancing the profession of animal control in the State of Florida.

LIFETIME ACHIEVEMENT AWARD NOMINATION FORM:  This form must be included with nomination 2024 FAPAA Lifetime Achievement Award

NARRATIVE OF ACCOMPLISHMENTS/PERFORMANCE:  Please include separately, along with any supporting documents, letters of recommendation, etc., and submit with this form.

PHOTOS: Mandatory requirement for all awards. Please include at least one (1) quality JPEG image for use during the awards presentation.

DEADLINE: All nominations must be received no later than 5:00 p.m. on December 31, 2023.
