OUTSTANDING COOPERATIVE TEAM ACHIEVEMENT

BASE CRITERIA for Cooperative Team Achievement award:

1. Must be a current member of FAPAA, either a Regular Member or Associate Member.
2. Must demonstrate the success of a team effort to accomplish a stated goal. The team can be internal (a staff team effort), or external (working with other agencies or organizations).

OUTSTANDING COOPERATIVE TEAM ACHIEVEMENT OF THE YEAR NOMINATION FORM:  This form must be included with nomination 2024 FAPAA Outstanding Cooperative Achievement

NARRATIVE OF ACCOMPLISHMENTS/PERFORMANCE:  Please include separately, along with any supporting documents, letters of recommendation, etc., and submit with this form.

PHOTOS: Mandatory requirement for all awards. Please include at least two (2) quality JPEG images for use during the awards presentation.

DEADLINE: All nominations must be received no later than 5:00 p.m. on December 31, 2023.
